Appointments
If you are a new patient or you have not seen us for over a year then you will need a referral to arrange an appointment.
You may obtain a referral from your family doctor, walk-in clinic, emergency department or any other physician.

On your first visit to the clinic you should bring the following items:
- A valid OHIP card.
- Any recent blood or urine results, including PSA, available to you.
- Any imaging results, such as CT, MRI, ultrasounds performed in the last 6-12 months available to you.
- List of all prescriptions and over-the counter medications you are currently taking.
